Rybak Aggregate, LLC. · Flash burns (electric)

Employee was working on a conveyor and did not follow proper company procedures of lock out tag out before disconnecting a hot wire and received a flash burn to his left hand. Flash burn was treated with first aid procedures, was released and able to return to work
