Contractor EE moving overburden from one area of pit to another was descending the SW ramp empty when lost control of the Terex TA35. The truck drove into the W.berm/boulders which redirected him to the E.berm where truck drove up and thru berm resulting in truck upset & landing on the level below. EE was seen at hospital and released with minor injuries.

Employee had pulled on a wrench to tighten a hex head nut. He then took a last pull to cinch tighten the nut, when the wrench slipped off the grade 5 nut. The employee fell back to the same level injuring buttock.

EE WAS POSITIONING A SWING WRENCH WHILE ANOTHER EE WAS APPLYING FORCE TO THE WRENCH WITH A SLEDGE HAMMER. THE HAMMER GLANCED OFF AN OIL LINE, STIKING THE (FIRST) EE'S HAND. THIS RESULTED IN A FRACTURE TO A METACARPAL BONE IN THE RIGHT HAND.
EMPLOYEE WAS OPERATING THE DRILL. HE OPENED THE CAB DOR AND WHEN CLOSING IT, HE CAUGHT HIS LEFT INDEX FINGER ON THE DOOR LATCH. THE LLACERATION REQUIRED 4 STITCHES.
AT AN UNKNOWN TIME, PRESUMABLY EARLY MORNING HOURS, A 19-YEAR OLD MALE TRESPASSER TOOK HIS OWN LIFE BY JUMPING OFF A 70-FOOT HIGH WALL AT A REMOTE AREA OF THE LIMESTONE QUARRY. THE TRESPASSER CROSSED OVER A 5-FOOT HIGH FENCE AT THE POINT THAT THE POINT THAT POLICE INDICATE HE ENTERED MINE PROPERTY. CORONER JUDGES THE INCIDENT AS SUICIDE.

NON EE , NON CONTRACTOR TRUCK DRIVER PICKING UP A LOAD OF STONE FOR A CUSTOMER WAS DUMPING THE OVERWEIGHT PORTION OF HIS LOAD AT A STOCKPILE WHEN A SECTION OF THE PILE SLUFFED OFF COVERING THE DRIVER UP TO HIS NECK AND PINNING HIM AGAINST THE TAILGATE PAN FOR 15-20 MIN. HE NEVER LOST CONSCIOUSNESS. THE DRIVER WAS RESCUED BY VULCAN EE AND IS BEING TREATED AT MERCY HOSP. FOR PELVIC
HE BELIEVES HE WAS ROTATING THE DRILL STEEL ON THE DRILL RIG WHEN HE LEANED FORWARD AND HIS SHOULDER HIT THE LEVER THAT SPINS THE DRILL. HIS RIGHT THUMB WAS CAUGHT BY THE DRILL STEEL AND THE U PPER PART OF THE RIGHT THUMB WAS TORN OFF AND THE SKIN OF THE RIGHT THUMB WAS TORN OFF DOWN TO THE HAND.
