EMPLOYEE BENT OVER TO PICK UP A BEAM (10") THAT WAS 30'L AND WEIGHED APPROX 65LBS. WHEN THE PICKED IT UP. HE THREW IT BACK DOWN AND COMPLAINED OF BACK PAIN.
EE WAS LIFTING A PIECE OF STEEL AND FELT A BURNING SENSATION IN HIS BACK. EE ON RESTRICTED DUTY TO PREVENT REOCCURRENCE.
EE WAS TIGHTENING BOLTS ON A CONVEYOR WHEN THE SOCKET SLIPPED OFF THE NUT CAUSING HIM TO BUMP HIS ARM ON A NEARBY HANDRAIL POST. EE WAS GIVEN A PRESCRIPTION FOR TYLENOL #3 WHICH IS THE ONLY TH ING CAUSING THIS TO RESULT IN A REPORTABLE INJ.
EE WAS LOADING AN EMPTY OXYGEN BOTTLE INTO A BASKET. THE OXYGEN BOTTLE SLIPPED AND SMASHED THE EMPLOYEE'S FINGER AGAISNT THE HANDRAIL ON THE BASKET.
