Pete Bajo: 1 recorded fatality Metal/Non-Metal

Ee loading a 16' bolt into resin head and centralizer. Coworker pressed button, closing centralizer on ee's hand. Ee seen at clinic, laceration to pinky. Pinky buddy taped and laceration closed with steristrips. On follow-up, antibiotics given for infection, finger redressed and buddy taped. Ee working full duty.

EE was taking wire to a heading. Reports they were walking downhill, stepped wrong, straining ankle. Seen at ER, given brace and crutches, and released to return to work restricted.

EE was chipping concrete while in a batch truck. After they got out of truck, they felt stinging sensation on buttocks. They had somehow gotten cement material in underwear/under clothing causing burn to buttocks. Seen at Health, referred to ER for removal of cement. Released to restricted duty, working restricted duty.

EE reported on 4/21 that on 4/19 and then again on 4/21 they smelled ammonia in their work area. On 4/21 they reported cough and burning in their lungs. Gas readings from their monitor did not show overexposure. Seen at clinic, diagnosed with possible chemical exposure/pneumonitis and released to restricted duty, no working underground pending follow-up. Site offered modified duty.

EE parked mucker to set up in auto. Took 4 steps up a slight incline and felt a pop and instant pain in left calf. Seen at clinic, diagnosed with a sprain, and released to modified duty, seated work only, on crutches. Does not appear to be an injury by accident.

EE was shoveling dirt out of the truck in the shop. They threw a shovelful of muck and felt their neck pop. They report feeling pain and numbness. Seen at ER, diagnosed with pain, released to full duty. On follow-up 10/24, EE referred for physical therapy making this reportable for treatment.

Employee was climbing into the cab of the nipper flat deck (open cab). As EE was entering, EE's mine belt accidentally got caught on the door, causing the door to be closed behind EE. EE's thumb was caught between the door and the door frame as EE turned to sit in the seat.

Employee was unloading wire mesh out of the mucker bucket. EE lifted the corner of the wire mesh when EE slipped on another piece of mesh panel, causing EE to fall and catch their hand on the mucker bucket causing injury.

EE reports being loaded at loader 46, and being "shook hard" when the loader was leveling out material. Reports neck jolted sideways and feeling immediate pain and migraine. Video review does not support verbal report. EE reporting severe pain at follow-up appointment and put off work pending follow-up.

Ee driving tractor underground, reports being unable to downshift and slow tractor. Ee reports ribbing the tractor and jamming right wrist into steering wheel. X-rays negative for fracture. Ee released to light duty site can accommodate.

Ee was working from a man basket that was boomed out to remove hose. The basket bounced, and the ee's right ring finger was pinched between the basket and pipe hard line, causing a crush injury and amputation of the tip of the right ring finger. Orthopedic specialist in SLC did a revision amputation to the first knuckle. 60 days permanent disability.

EE was getting ready to unload bolting supplies from a mucker bucket. The bolts and wire started to fall, EE tried to move out of the way, but bolts fell on left foot. Seen at hospital, multiple fractures to left foot. EE referred for orthopedic surgery consult.

While hooking up to off-load cement, the product hose slipped off the fitting, came back and hit driver in the mouth causing a contusion inside EE's mouth on EE's lower lip. It wasn't deep enough for stitches but EE was prescribed an antibiotic.

EE was loading rounds at the face. A small piece of debris came off the face, striking EE's left wrist. EE reported and did not seek treatment. When the pain did not subside, EE saw a doctor on 11/30/21 and was diagnosed with a chip fracture and released with restrictions that do not preclude EE from performing all the essential functions of the role.

Ee was working on a loader. The hood cover fell and slammed on Ee's thumb. Ee was seen at a clinic diagnosed with an avulsion fracture, and released to full duty.

EE was lowering gate, it got stuck on the rib, and glove caught on a wire. While pulling out the glove, the gate fell, striking the EE on right collarbone. EE was diagnosed with a fractured right collarbone and will be having surgery to repair on 1/22.

Two employees were changing cutting edges on the 988 surface loader in the east bay of the Pete Bajo surface shop. The new cutting edge was resting on top of the old cutting edge on the floor, supported by a bolt on one side. The cutting edge slid, crushing the ee's left hand. Diagnosed with a fracture to the left tip of the thumb, and laceration. Put on restricted duty.

While performing work on the Pug mill conveyor and tail pulley, an employee was utilizing an air lance to remove material build up. Ice dislodged from the wand causing the employee to lose control of the wand. At some point the employee injured the left wrist which resulted in multiple fractures.

Employee was getting on a 988 surface loader using three points of contact. While stepping up onto the bottom step, the employee felt a pop in the left shoulder. The employee sought medical treatment and was unable to return to work.

Employee was closing the lid lock on a batch truck in the 1285 level.  While striking the lid lock with a wrap wrench in right hand, the wrench struck the employee's left middle finger. The employee sought treatment and received 17 sutures to close the wound.

Employee was walking across a hard packed icy parking lot and slipped falling on right side and back Employee sought treatment on 1/25/17 that identified a fracture to the 7th rib.

At approximately 10:00 p.m., during the post blast inspection of the 1330-705 heading, a fall of ground was identified. The fall of ground ranged from 7 to 13 feet high and worked back from the face approximately 50 feet. There were no injuries or damage to equipment as a result of this event.

Employee had been mounting/dismounting a tractor throughout the shift. Towards the end of the shift, the employee reported pain in the left knee. The employee sought medical treatment and was unable to make the next scheduled shift.

EE was in the process of depressurizing a discharge line on a portable pump when a reducer on the line failed. Discharge line contacted the ee and the ee was knocked to the ground at which time some material entered ee's right eye. This resulted in a corneal abrasion and a subsequent sensitivity to light which did not allow the employee to return to work.

An employee was in the process of loading knee holes in the 4640-120 when loose rocks came off the face and cracked the employee's hardhat. Sharp edges from the damaged hardhat contacted the back of the employee's head causing a laceration. The laceration required staples to close.

Employee was in the process of loading a face round when some material came off the face and struck the employee on the back of the head and shoulders. The employee was evaluated at a medical facility. Due to bruises and abrasions, the employee was released on restricted duty.

Employee was spraying shotcrete in the 1330 AXS. The employee was wearing safety glasses and goggles and while spraying, some debris came onto his goggles. The employee attempted to wipe them off and some debris went around the safety glasses and on top of the right eye.

While in a manbasket being lowered to the ground with a forklift, the employee pinched his left pinky finger between the basket and the fork frame. This event resulted in three sutures.

Employee was working with another mechanic and was lining up a safety pin for the tailgate of a haul truck and his index finger was pinched between the end of the pin and the under side of the haul truck bed crushing the tip of the finger.

While trying to shotcrete the Southeast Decline, operator had lost air pressure and had his nozzle plug up. Sprayer operator shut off the air and started disconnecting the air and accelerator valve at the end of the boom when the energy was released it sprayed the operator in the face.

While adjusting the air doors the Left hand was smashed between the adjustment arm and the header of air door.

In the Upper Fence Decline drift at the Pete Bajo Mine a ground fall occurred. The failure occurred as the drift was broken through to re-connect to an existing drift.

While transporting employee's from Pete Bajo back to the dry at the end of day shift, the bus hit a bump and jarred the bus occupants around. One employee reported soreness in his back and requested to seek further medical attention.

Mechanic was laying under the Getman Mobile batch mixer truck getting ready to work on the transmission when he pulled the floor jack to position it under the transmission and felt a sharp pain in his back.

Employee stepped off his haul truck and twisted his right ankle. The employee was given a restricted duty release.

Employee was stepping off the crew bus at the dry at the end of shift and sprained his left ankle which resulted in a restricted duty release.

EE had a fall of ground in the 5170-210 Drift. The heading was in the bolt cycle. Operator noticed ground movement then he pulled the bolter out to a safe location and heard the ground fall.

Driver and passenger where driving down haul road to Pete Bajo Pit from Leeville. When trying to make left hand turn the truck rolled onto the passenger side.

Driver and passenger were driving down the haul road to Pete Bajo Pit from Leeville. When trying to make a left hand corner the truck tipped on the passenger side.

When removing lid on remix truck to wash out it fell hitting his finger.

Shift supervisor was called to the 5265-516 by the jammer operator to look at the condition of the heading. The last wire and bolts were hanging in the heading that was being jammed up with cement. The shift supervisor thought it was a ground fall because the bolts were hanging out.

Shift supervisor was doing his daily round and went into the 5432-110 and found a ground fall had taken place. The area was approximately 30' long and 12' high and 15' wide. No equipment or personnel were working in the area.

Employee was placing a rock bolt in the carousel of the bolter when he apparently dislodged a rock sitting on the carousel. The rock fell and struck him on the hard hat causing a neck strain. At this time he was issued a full duty release. On February 4th he returned to the Dr. and was issued a restricted duty release. He was on restricted duty for four (4) shifts.

Employee was taking an electrical cable out of a cab hook which attached to the rib of the drift. The cab hook spun resulting in a laceration between the index finger and the middle finger on his left hand. It required about 7 sutures to close.

Employee was cutting the insulation off of an electrical cable when the razor knife slipped causing a laceration of left hand between his thumb and index finger. The laceration required 8 sutures.

Removed clamp on air line without bleeding off pressure.

Step off pick-up truck bed and sprained ankle.

After cleaning shot Crete re-mix truck he was replacing the lid, when he caught his thumb between the truck and the lid, cutting his thumb. His received sutures.

Fell from a man-basket fracturing his left leg.

Vega employee was driving a 10 Wheel dump truck following a TIC employee up a hill at Newmont. It was slippery and the truck lost traction and slid down on its side. Vega employee was taken by Newmont ambulance to Northern Nevada Regional Hospital.
