Workers were working on the fast tracks. Worker drove a loader next to the tracks so they could stand on the bucket. Worker pulled the loader around and set the parking brake. He climbed on top of the bucket to hook the chain with a come-along. The loader rolled forward, worker jumped onto the fast track and his left foot was caught between the loader and bucket.
