OPERATOR WAS DESCENDING FROM 980F WHEEL LOADER & MISSED THE BOTTOM STEP ON THE LADDER. SHE FELL BACKWARDS ONTO THE CONCRETE, HITTING HER HEAD.

A 76 HOLE SHOT WAS BEING SET OFF IN THE PIT. THE INJURED PERSON WAS STANDING APPROX. 410' FROM THE BLAST SITE. FLY ROCK FROM THE BLAST STRUCK THE PERSON CAUSING THE INJURIES.

SCALE PLATFORM HAD BEEN REMOVED FOR REPAIRS. WHILE PIT WAS EXPOSED, VACUUN TRUCK WAS TO CLEAN OUT FLOOR OF PIT. HE WAS ASSISTING WITH CLEAN-UP. HE WAS ENTERING PIT, STEPPED ONTO BLOCKING HOLDI NG UP SCALE BEAMS WHEN THE BLOCKING BROKE AWAY AND HE LOST HIS BALANCE AND FELL INTO THE PIT. HE PUT HIS RIGHT ARM UP TO PREVENT HITTING HIS HEAD ON BEAM AND BROKE HIS FOREARM JSUT ABOVE HIS
