EMPLOYEES WERE PREPARING CONVEYOR FOR TRANSPORT. WHILE PLACING MATERIAL ON THE CONVEYOR WHICH WAS BEING HELD IN POSITION BY A LOADER THE CONVEYOR ROLLED FOWARD AWAY FROM THE LOADER CAUSING THE TOP END TO FALL ON OVER EMPLOYE CAUSING MULTIPLE INJURIES.
