I WAS WALKING GROUND PUTTING AWAY TOOLS LOCKING UP THE CRUSHER WHEN I STEPPED ON SOME ROCK WITCH ME OFF BALANCE TWISTING MY ANKLE AND FALLING DOWN THRU.

EMPLOYEE WAS STANDING ON CATWALK WHEN IT SUDDENLY GAVE WAY AND CRASHED APPROX. 6 FT TO THE GROUND. THE CATWALK GAVE WAY BECAUSE THE BOLTS HAD WORN DUE TO VIBRATION OF CRUSHER.

SUBJECT WAS ATTEMPTING TO JAR SOME LARGE ROCKS LOOSE SO THAT THEY WOULD SLIDE DEEPER INTO THE CRUSHER AND BREAK UP. THE ATTEMPT WAS BEING DONWE BY USING A LARGE METAL BAR. BAR FLIPPED BACK ST RIKING SUBJECT ABOVE LEFT EYE ON FOREHEAD. SUBJECT WAS NOT WEARING HIS HARD HAT OR SAFETY GLASSED IN VIOLATION OF CO. SAFETY RULES.
