BACK SEENED TO 6E HURT IN 2-84 FROM LOADING A SHOT. EMPLOYEE WOULD NOT GO TO DR. NO WORK WAS MISSED OVER THE LAST 3 MONTHS BACK COMPLAIN STARTED AND 5-12-86. HE WENT TO DR AND A SPUR WAS FOUND ON VETBRA CAUS3NG PRESSURE ON A NERVE. DR SAY BOUNCING ON THE LOADER COULD HAVE CAUSED THIS AND IT DOES KEEP IT INFLAMED.

AFTER SHIFT THE DOZER OPERATOR WAS FILLING HIS DOZER THE FUEL NOZZLE WOULD NOT COMPLETELY CLOSE OFF & DIESEL FUEL IGNITED IN HIS FACE. THE OPERATOR HAD BURNS TO HIS FACE & LEFT HAND & THE O NLY DANAGE TO THE DOZER WAS BURNED SEAT & A COAT LAYING BESIDE THE SEAT.
