April 15, 2025 WA · Metal/Non-Metal rotary bucket excavator operator SLIP OR FALL OF PERSON
Thayer Excavating LLC dba Riverbottom Rock · Fall to the walkway or working surface

The employee climbed off the excavator to relieve themselves. Employee fell when standing on the ground and hit their shoulder/back of their head. Ended with bruising on the shoulder and a hairline fracture of the L2 vertebral. No treatment provided or needed. This was the employee's last day of work.
