WHILE DIGGING INTO A HIGH WALL THE BANK GAVE WAYAND BURIED THE LOADER WITH MATERIAL CAUSING INJURY TO HIS BACK.

WHILE ADJUSTING THE JAW TIGHTENING THE NUTS ON THE SPRINGS. THE EMPLOYEE SMASHED HIS HAND ON THE CATWALK.
WHEN HE WAS SHOVELING MATERIAL FROM UNDER THE CRUSHER JAW. WHEN HE STOOPED AND TURNED TO SHOVEL OUT THE MATERIAL HE FELT A SHARP PAIN IN HIS UPPER BACK AREA.
WHILE DIGGING ROCKS OUT OF THE JAW CRUSHER. HE LIFTED A LARGE ROCK AND THREW IT UP ON THE GRIZZLY BARS THIS CAUSED A PILE OF ROCKS TO FALL ON HIS HAND.
WHILE HE WAS REACHING UNDER THE CONVEYER BELT WITH THE RAKE, HE SUDDENLY FELT PAIN IN HIS CHEST. HE THEN STOPPED FOR APPROX 5 MINUTES, THEN CONTINUED BUT AGAIN HE FELT SEVERE CHEST PAIN.
EE WAS TRYING TO KNOCK THE LOCK LOOSE ON THE JAWDYES WITH A BAR. THE BAR SLIPPED, BOUNCED & CUT HIS RIGHT HAND FINGER.
