EMPLOYEE WAS DI8MANTELING THE DRIVE SHAFT BETWEEN THE HAMMER MILL AND POWER UNIT. WHILE LOOSENING ABOLT, THE RACHET AND SOCKET SLIPPED OFF THE BOLT CAUSING EMPLOYEE TO LOSE HIS BALANCE. HE F ELL FORWARD, ST9ICKING HIS MOUTH ON THE DRIVE SHAFT, BREAKING ONE THOOTH AND BREAKING HIS PARTIAL PLATE.
