Pyramid Materials Inc. · Struck by flying object

Private contractor dumping dirt on our dumpsite broke down, requiring to be pulled out to a safe location to work on truck. Chain broke and hit windshield shattering glass on truck Driver Causing minor cuts/scratches to Right ear and right cheek. No other injuries. Injury not requiring medical attention.

EE was found slumped over EE's steering wheel. Another EE was first to find EE and called 911. Another EE showed up and they checked for pulse then removed EE from the truck, then the ambulance arrived. The paramedics could not revive EE and told us that it was cardiac arrest in which EE died.

Rock bounced off vibrating screen deck at crushing plant, striking employee on top of hardhat. Head was lacerated at crossing of hardhat suspension. Employee received staples on top of his head and was advised to beware of concussion symptoms.
