The victim was being task trained to operator a haul truck and lost control of the truck while backing down a ramp. The truck ran through a berm, overturned, and fell approx. 30 feet to the bench below.

Incident Report On Thursday May 30th around 10:20am IE switched from driving 410E off-road truck to 972 CAT loader. Moments after an EE found IE with the bucket down parked on the side of the road about 50 yards from where IE got in the loader.

employee was working on a semi truck with hand tools when he was trying to loosen a nut on a U bolt resulting in torn bicep on his right arm.

walking around plant when he twisted his left ankle. Employee was taken to Emergency Room, put on crutches for twisted ankle. He did not miss any days of work as he was placed on office work. Did not report this to Workman's Compensation. Bill was pain by company.

OFF ROAD TRUCK WAS HAULING LIME ROCK FROM QUARRY & HIT A ROCK W/TRUCK WHEEL CAUSING STEERING WHEEL TO SNATCH LEFT WRIST. EE DID NOT WANT TO SEEK MEDICAL ATTN. THE DATE OF ACCIDENT, WHEN HE RET URNED TO WORK ON MONDAY,12/29/03, HE SAID HE NEEDED MEDICAL ATTN. HE WAS DIAGNOSED WITH A SPRAINED WRIST & RETURNED TO WORK THE FOLLOWING MORNING.
