EE stated he was placing a conveyor belt tail roll in place. Come-a-long chain came loose- caught both hands between tail roller & steel plate. Fracture to little finger left hand, ring finger, r. hand, both fingers required stitches from lacerations. Started missing work 6/8/07.

IT WAS REALLY MUDDY THAT DAY. HIS FEET BECAME TANGLED. HE LOST BALANCE AND FELL OFF THE TRUCK BREAKING HIS HIP(RT).
