Installing attachments to Hi-Wall Miner. Employee had finger between Miner and attachment. Cut tip of finger #4 (ring finger).

Employee got too close to berm on right side striking berm, losing control & crossing road striking left berm before regaining control of rock truck. Hit right arm on glass causing laceration. He bruised hip on seat frame. Employee told company official & foreman he was driving too fast. Employee tested positive on drug test.
