Southwest Rock Products LLC · Struck against a moving object

The employee was driving a haul truck, moving material around the plant,hit a rock in the roadway. The employee was not wearing a seat belt and as a result of hitting the rock, caused the truck to bounce employee up in the seat. When employee landed back in the seat, hurt back. This incident did not become a reportable injury until 5/9/2016 when lost time occurred.

Southwest Rock Products LLC · Caught in, under or between a moving and a stationary object

EE Directed the loader into position to load screen into bucket. signaled for loader to stop. walked in front of loader to drag screen into bucket when the operator lowered the bucket to the g round and EE's foot was under bucket tooth. crushing his left foot big and second toe. This incident caused both of the tips of his toes to be removed by the doctor.
