INJURED WAS STANDING ON TEH FEEDER AND THE WIND WAS BLOWING AND A PARTICULE OF DUST BLEW IN HIS EYE. THIS HAPPENED AS INJURED WAS OVERSEEING EQUIPMENT. HE WENT TO THE HOSPITAL LATER TAHT EVE NING TO HAVE IT CHECKED.

INJURED WAS CLEANING OUT THE FEEDER ON PRIMARY CRUSHER ANOTHER EMPLOYEE WAS DRIVING THE LOADER.HE SAW INJURED BUT DID NOT REALIZE HIS AHND WAS ON THE SECOND BAR AROUND THE TOP OF THE FEEDER AS INJURED WAS TRYING TO REMOVE CHAINS FROM AROUNDA LARGE STUMP IN THE FEEDER OTHER EMPLOYEE LET THE BUCKET ON THE LOADER DOWN AGAINST INJURED'S RIGHT HAND SMASHING THE INDEX FINGER.

EMPLOYEE WAS SHOVELING DIRT WITH A HAND SHOVEL FROM UNDER THE CONVEYOR BELT. THERE WAS SNOW ON THE GROUND AND IT WAS SPITTING SNOW. I STARTED TO STEP RIGHT WITH MY RIGHT FOOT, SLIPPED AND FELL TO THE GROUND. DOCTOR'S REPORT SAYS SPRAINED RIGHT ANKLE.

THE INJURED WENT INTO THE TOOL TRAILER TO GET WRENCH AND PUMP. HE DENIES SPILLING OIL AND GREASE ALL OVER THE SHED. AT ANY RATE, HE HAD GREASE ALL OVER THE FEET, STATRED DOWN STAIRS THAT GO O UT OF TOOL TRAILER AND FELL LANDING ON HIS REAR SCRAPED BOTTOM ARMS. ALSO A ACUTE BACK SPRAIN.
