THERE WAS VERY STRONG WINDS W/SNOW BLOWING. EMPLWAS GOING OUT THE DOOR WHEN THE WIND CAUGHT IT AS HE WAS STEPPING ON THE STAIRS & CAUSING HIM TO SLIP & FELL TO THE GROUND APPROXIMATELY 4'.
EMPLOYEE WAS CLEANING UP AROUND THE CONVEY0R AND LEANED OVER TO FAR, STRIKING HIS JAW ON THE SIDE OF THE CONVEYOR AND IT BROKE HIS TOOTH.

WHILE TAKING OFF BACK GUARD ON CHUTE, IT RECOILED AND THE EDGE OF THE ANGLE IRON STRUCK EMPLOYEE IN THE MOUTH, BREAKING HIS TOOTH. HE HAD TO HAVE HIS TOOTH RE-CAPPED.
WELDING WEAR PLATES ON DOZER BLADES ENTIRE DAY DURING MAINTENANCE DAY FOR CRUSHER. EYES BECAME VERY RED & STARTED WATERING LATER THAT SAME NIGHT.
