EE WAS STANDING ON TOP OF IMPACTOR, IT WAS SHUT DOWN. HE WAS TURNING ON WATER AND GAVE THE SIGN TO START IT UP. AT THAT TIME METAL PLATE INSIDE BROKE. THE IMPACT OR A WEDGE HIT EE IN THE BOTTO M OF THE FOOT OR ANKLE. EE HOPPED OVER TO BRAKE AND SHUT DOWN MACHINE. OUR FIRST RESPONSE PROVIDED 1ST AID AND TRANSPORTED TO HOSPITAL.

EE AND RUST TRACKTOR MECHANIC, WERE WORKIGN UNDER A HOISTED BUCKET OF 980F LOADER ON VALVE BODY ASSEMBLY WHEN THE HOIST GAVE WAY CATHCING EE BETWEEN HOIST ARM AND LOADER TIRE, CRUSHING HIM. W AS TAKEN BY AMBULANCE TO THE HOSPITAL IN SNATA ROSA, FOUND BROEN RIBS AND BRUISED LUNGS. WAS THEN TRANSFERED TO ALBUQUERQUE HOSPITAL WHERE IT WAS DISCOVRED THAT HE ALSO HAD A FRACTURED SCAPUL

EMPLOYEE WAS ANTIFREEZING AIR LIENS ON EQUIPMENT, POURED TANNER GAS IN AIR VALVE AND OPENED VALVE WHICH ADDED AIR PRESSURE, RESULTING IN GAS BLOWING IN HIS EYE. THIS CAUSED HIM TO MISS TWO DA YS OF WORK.
