THE EMPLOYEE WAS REPLACING A ROLLER, IN A ROLLER FRAME FOR A CONVEYOR. THE FRAME WAS ON THE GROUND. THE EMPLOYEE WAS PULLING UP ON THE ROLLER WITH A BAR, WITH A HOOK END. WHEN THE ROLLER CAME FREE, THE EMPLOYEE'S HAND AND BAR CAME UP STRIKING HIM IN THE MOUTH AND DAMAGED HIS TEETH.

THE EMPLOYEE WAS CHANGING BUSHINGS ON THE CONTROL LINKAGE OF THE BOBCAT LOADER. THE EMPLOYEE HAD RAISED THE OPERATOR'S CAB AND BLOCK IT UP WITH A BOARD. WHILE CHANGING THE BUSHINGS THE BOARD C AME OUT AND DROPPED THE CAB DOWN ON THE EMPLOYEE'S BODY. AFTER BEING EXAMINED AT THE WINNEMUCCA HOSPITAL ER, THE DOCTOR TOLD ME THE EE WAS BADLY BANGED AND BRUISED.
