EMPLOYEE RECEIVED A "BOIL" ON THE LOWER BACK FROM THE LOADER SEAT. EMPLOYEE SAID THE SEAT IRRITATED THE SPOT. THE SPOT BECAME VERY TENDER.

THE JAW PLANT WAS SHUT DOWN AFTER A ROCK WAS JAMMED IN THE CONVEYOR. THERE WERE 2 WORKERS, ONE USED THE BOBCAT TO PUSH THE ROCK AND THE OTHER WORKER STARTED REMOVING THE ROCK. INJURED CRAWLED OUT OF THE BOBCAT TO SEE WHAT WAS GOING ON. HE BENT OVER AND TOOK HIS HARD HAT OFF TO SEE. THIS IS WHEN THE OTHER WORKER THREW THE ROCK BEHIND HIM & HIT EE IN THE HEAD. HE RECEIVED A FEW STITC

EE WAS USING A PRY BAR TO PRY A ROCK OUT OF THE JAW WHEN THE BAR LET LOOSE & HIT HIS LEFT INDEX FINGER AGAINST THE WALL OF THE JAW. INJURY WASN'T CHECKED OUT BY A DOCTOR IMMEDIATELY SINCE EE THOUGHT INJURY WAS ONLY A BRUISE.

A BARREL OF ANTI FREEZE WAS BEING LIFTED DOWN FROM A TRAILER WHEN EE LOST HIS BALANCE AND FELL TO HIS FEET, APPROX 3-4FT FROM THE GROUND. EE DIDNOT SEE THE DR UNTIL 9/15/97 (HIS CHOICE) A BOBC AT WAS USED TO LIFT ANTIFREEZE DOWN.
