EE was assisting with removing a rock from the jaw crusher. The forklift hit the work platform guardrail which hit EE's hard hat. The hard hat slid down onto EE's face and nose leaving a small abrasion and broken nose.

EE was prying a rock out of a chute with a digging bar using a cross member of the chute for leverage. When the rock dislodged, EE smashed finger between the cross member and the digging bar.
EE was hammering on a bearing with a sledgehammer attempting to replace a bearing. The bearing was on a shaft with a sheave next to it. While swinging the hammer at the bearing EE's hand contacted the sheave. EE smashed EE's pinky finger. EE drove to a local urgent care facility for stitches. EE received 3 stitches in EE's right pinky finger.
Miner was injured trying to remove a rock from a jaw crusher. An excavator was used to hoist the rock. The excavator moved unexpectedly and struck the miner standing on the work platform of the jaw. Miner experienced a head and shoulder injury.

EE was installing flexcos on a conveyor. EE neglected to ensure the conveyor was locked out. The conveyor started without warning and EE was deposited into a feeder that was not in operation. EE fell 4 feet to the chute on the feeder and rolled/slid into the feeder. The exact severity of injuries is unknown currently.
Employee was attempting to remove/manipulate a rock from the jaw crusher. Pinched finger between railing and a "digging bar".

Employee was cleaning rocks from around a tail pulley through the guard. The employee decided to remove the guard while the belt was moving. EE reached in to remove rocks and pinched EE's finger in the self-cleaning tail pulley.
Employee was hoisting a rock out of a jammed jaw crusher with a chain and rock came loose and rolled down on employee's left leg scratching EE's shin and hurting leg

Was working to replace a bearing on conveyor head pulley. Employee had to remove gear box to get to bearing and bearing shaft had froze up so took a hammer to hit the shaft to free the bearing and piece of metal flew off hitting employee in the right hand. Employee had removed gloves 15 min. earlier to work on the bearing.
Injury occurred when it was night time. Employee was working on jaw crusher and went to mechanic truck to get a bolt and nut. Employee tripped and used arm to catch the fall and ended up straining right shoulder.

Windy day and EE mentioned he got something in his eye when he got into pick-up with supervisor to warm up. Supervisor replaced safety glasses with goggles and he continued to work. He did not show up to work the next day nor called in. He came into office 11/30/06 stating he went to dr. Has follow up 12/6/06.

Was cleaning the lid on the Canica VSI crusher. When he went to climb down his foot slipped on the rail and he twisted his knee.
TEARING DOWN THE CRUSHER TO MOVE TO A NEW LOCATION. EMPLOYEE WAS CUTTING SOME ROPE & THE KNIFE SLIPPED, CUTTING HIS FINGER.
