Eastern Oklahoma Quarries LLC · Struck against a moving object

Employee was driving an end dump and complained of chest pains and numbness in left arm. I called 911, EMT's examined EE and took EE to the Hospital. Dr. at the Hospital determined EE was having muscle spasms in chest. EE was released from the hospital that afternoon. Employee requested 2 or 3 days rest.
