EE was working on a conveyor. EE tripped on the tongue and fell along side of the material hopper catching the inner part of EE's thigh on the hopper and received a cut. EE was taken to the hospital, received stitches and returned to work.

EE was replacing blocks and shims under a plant support. EE's thumb was under a shim when the jack was lowered. EE was taken to the hospital to have 4 stitches in right thumb.

FROZEN SHOT ROCK PLUGGED PRIMARY JAW CAUSING DRIVE BELTS TO BURN OFF. INJURED WENT TO QUARRY TO HELP CLEAN OUT JAW. WHEN LIFTING SHOT ROCK OUT OF JAW LOWER PATTELA TENDON BROKE IN RIGHT KNEE.
