EMPLOYEE WAS CLEANING MUD OUT OF SCREENS. THE CLEANING PLANT WAS RUNNING. A BOLT FELL ONTO CATWALK. WHEN EMPLOYEE LEANED OVER TO PICK UP BOLT, HIS HARD HAT FELL OFF. WHEN HE RAISED UP, HE HIT HIS HEAD ON THE EDGE OF A STEEL PLATE.
