EMPLOYEE WAS PULLING REBAR OUT OF A PULLY WHEN THE GLOVE SHE WAS WEARING WAS PULLED OFF CAUSING HER TO GET A CUT ON HER RING FINGER. EMPLOYEE DROVE TO HOSPITAL IMMEDIATELY WHERE SHE RECEIVED 3 STITCHES.

EE WAS DRIVING ROCK TRUCK, HAULING MATERIAL TO ASPHALT PLANT WHEN SHE WAS GETTING OUT OF TRUCK, SHE SLIPPED ON CHAIN ON BOTTOM RUNG OF THE LADDER-SHE TWISTED HER RIGHT KNEE. ACCIDENT HAPPENED APPROX. 400 FEET FROM CRUSHING OPERATION ASPHALTPLANT OWNED BY AMERICAN ASPHALT.
