EE WAS EXAMINING A NOISE AT THE PULLEY HEAD MOTOR OF A CONVEYOR BELT. EMPLOYEE LIFTED THE GUARD TO EXAMINE ASSEMBLY AS IT WAS RUNNING. EE LOST HIS FOOTING, HE REACHED FOR A HANDHOLD. HIS HAND WAS GRABBED BY THE V-BELTS AND HIS FINGERS WERE MASHED BETWEEN THE PULLEY AND THE V-BELTS
