TRUCK WAS BEING LOADED WITH STOKER COAL UNDER THE TIPPLE WHEN THE TRUCK DRIVER DECIDED TO CLIMB VIA THE CAB AND FUEL TANK TO OBSERVE THE LOADING. AS HE WAS MOVING ACROSS THE FUEL TANK TO THE T RAILER HE SLIPPED AND FELL APPROX 3 TO 4 FEET TO THE GROUND. THERE WERE NO UNUSUAL CIRCUMSTANCES SUCH AS RAIN OR SNOW - WEATHER WAS COLD BUT CLEAR

A TRUCK DRIVER, NOT ASSOCIATED WITH THIS COMPANY, WAS LOADING CHUNK COAL. AS THE COAL WOULD COME OFF THE LOADING BOOM, HE WOULD ARRANGE THE COAL ON HIS TRUCK BY KICKING IT TO THE LOCATION HE D ESIRED. THIS IS DONE IN ORDER THAT THE COAL CAN BE SAFELY HAULED AND THAT THERE IS NO CHANCE A PIECE MAY FALL OFF WHILE ON HIGHWY. AS HE WAS ARRANGING THE LOAD HE SLIPPED AND FELL TO GROUND.
