EE was welding in the primary feeder, while welding EE suffered flash burn not knowing until the next day. Cause was possible flash reflecting off the feeder coming through the back of EE's welding hood.

EE was driving into work, when EE was pulling into the office parking lot at the Plant when EE's phone fell off the magnet holder in the air vent causing the phone to go between the floor and the accelerator pedal. When EE went to pick it up the vehicle veered into a rock blocking the electrical panel causing the vehicle to stop immediately.
