Employee claims 50# bag of sand fell from conveyor and struck him in the back. Employee did not report accident until days later after another accident. Suspect insurance fraud. Said lower back hurts.
Employee was leaving the parking lot at a high rate of speed (according to police report) when he crashed into the rear entrance pole gate. Employee suffered cut lip and bruises.

Employee was picking up a pallet from a pile that was chest high. He was picking pallet up, turning to the right to place pallet on ground for repair. (He did not notify supervisor of accident until 6/16/09 at 2:00 pm). Employee strained left shoulder when turning to the right.

Employee was stepping down off bag chair when her foot slipped on the round "shaft-like" step causing her to sprain her ankle.
Clearing underbrush with a bulldozer the employee was struck in the forhead by a broken limb causing a laceration which required 5 stitches.
Employee was climbing 2 foot retaining wall surrounding fuel tank to fuel up the front end loader. Employee lost footing and slipped, hitting left leg on wall. Required 4 stitches to close wound.

Employee climbed onto a conveyor to remove a jammed bag stopping the production line. Employee lost his balance and fell off of conveyor (about a 9ft drop) and landed on his shoulder and head. Employee suffered a contusion to the head / concussion, and sore shoulder.

Miner was clearing bags off of main line reject conveyor. Another miner was on top of cat walk and pushed a bag of dry mix concrete off of another conveyor. Bag of concrete fell and hit injured miner in left shoulder. Injured miner was seen at a medical clinic on 4/21/06 for pain in left shoulder.

Employee was coming out of break room door as forklift was entering warehouse. Forklift driver claims victim was in his blind spot and did not see him. Forklift bumped into victim. Contusion to knee, wrist, and lower leg.
Employee was picking up limb debris from Hurricane Dennis when he felt a sting in his back. Was sent to doctor. Given temporary alternate duty for 8 days for lumbar strain.

DRIVER WAS UNTARPING A LOAD OF BAGGED CONCRETE AT CUSTOMER'S STORE. DRIVER JUMPED DOWN FROM PALLET OF CONCRETE TO FLOOR OF FLAT BED TRAILER, TRIPPED ON TARP AND STUCK OUT HAND TO STOP HIS FALL. HE HIT HIS HAND ON THE FRONT BOARD OF TRAILER. FRACTURED CLOSE METACARPAL / RIGHT HAND.
EE WAS RE-STACKING A PALLET OF CONCRETE BAGS WHEN HE FELT A PULL IN HIS BACK. AFTER INVESTIGATING, IT WAS FOUND THAT EE WAS NOT LIFTING CORRECTLY AND FAILED TO WEAR HIS BACK BELT.
EE WAS TRAINING ON PALLETIZER AND DID NOT HAVE HIS HAND CLEAR OF THE SLIDE DOOR WHEN IT WAS CLOSED. THERE WERE 2 MEN TRAINING AND 1 TRAINER. TRAINER WAS UNAWARE EE'S HAND WAS NOT CLEAR. CONTUSION TO RIGHT HAND.
