EMPLOYEE HAD CL3MBED LADDER, WHICH IS A PERMANENT LADDER ATTACHED TO ANOTHER STRUCUTE, TO CHECK SCREENS TO SEE IF COAL WAS FLOWING PORPERLY. AS HE WAS CLIMBING DOWN HIS FOOT SLIPPED AND HE FE LL ABOUT 2 FEET3TO THE GROUND. WHEN HE REALIZED HE WAS FALLING HE TRIED TO CATCH THE LADDER AND SPUN AROUND AND HIT HIS HEAD ON THE SIDE OF THE LADDER.
