Wesco Minerals LLC · Fall from scaffolds, walkways, platforms

The employee was on a platfrom 9 ft high helping another employee when he said he "forgot where he was at," stepped backwards and fell to the lower deck. As he hit the ground all of his weight went into his right leg breaking it in 2 places and crushing his heel bone. As he fell he injured his left bicep with 3 scratches and bruising.

Wesco Minerals LLC · Struck against a moving object

EE was driving a Komatsu 400 haul truck. He went around as bend in the road and hit the right berm which cause the truck to jerk to the left side of the road and over the berm into the desert. He received 5 staples in his head, 4 stitches on nose, abrassion on forehead, concussion,left hip & right knee bruising and was confused pain and bleeding after the accident.
