In the No.4 Entry (primary escapeway) between x-cuts No.4 and No.5 going underground from the surface, there was a rock fall measuring 15' wide, 8' long, and 4 1/2' thick.

During the weekly exam of air counts a rock fall was discovered on 3/9/11 at 6:25am in the #4 intake air course. Approx 20' inby the mine fan. The rock fall is approx 10' wide X 8' long X 4' high.

EE stated that he was hanging his tie off to a bolt anchored in the roof. He twisted his body when he reached up to hang the shuttle car cable tie off, he felt pain in his neck and right shoulder.

The injured employee stated that he was installing a spill rubber at the No.2 head drive. He was between the head drive and backboard in a small area. He tried to turn around and he twisted his back.

A roof fall was discovered in the main return air course 55 crosscuts outby MMU 002-0 while making a weekly examination. The roof fall was approx. 4' thick 20' wide by 30' in length. The roof fall did not interfere with the air flow. The roof fall was cribbed and the travelway was moved to the other entry.

The injured employee was putting roof bolts into the roof, water was dripping from the roof and got the injured wet. While holding onto the drill steel, he felt an electrical shock in his arms and chest.

A rock fall was found by employee while making weekly examination of the return and the old worked out area of the mine. The fall was in the No 4 Panel, No. 5 entry, 9 crosscuts from the No 1 main entry. The rock fall was about 20 feet long, 20 feet wide by 4 feet thick. The No. 5 entry is cribbed off and is not traveled by the examiner

EE was crossing a conveyor belt line that was not in operation. For an unknown reason he stepped on top of the wet slick belt causing him to fall forward striking his right hand on a piece of rock breaking his right thumb.

Employee was helping the electrician put a lid on the power center. The lid dropped on employee's left hand index finger. It injured the area from the last knuckle up. The doctor later removed the tip of his finger from the last knuckle up.

A rockfall was discovered during a weekly exam in an isolated area 2400' underground in the #4 entry of five entries. The fall had no affect on ventilation and/or travel and measured approx. 70' long by 20' wide by up to 5' thick. All entrances to the area were cribbed and dangered off.

Injured was constructing a brattice when a block that he had stacked fell and struck him on the left index finger, fracturing it near the tip. He left the job at 3:00 pm that day and has not returned.

ABOUT 3:00 A.M. A SECURITY GUARD ON THE THIRD SHIFT WAS FOUND DECEASED SITTING AT THE DESK IN THE MINE OFFICE. LAST KNOWN CONTACT WAS AROUND 2:00 A.M. WHEN HE WAS TALKING ON THE PHONE WITH ANOTHER GUARD COMPLAINING OF CHEST PAINS AND BEING SICK. THE GUARD CALLED FOR AN AMBULANCE AND SOMEONE TO CHECK ON THE VICTIM. THE CORONER AT THE HOSPITAL RULED DEATH BY NATURAL CAUSES.
