EMPLOYEE CUTTING HOLE IN OVERSIZE CHUTE TO UNCLOG IT USING A WELDING/CUTTING TORCH. THE O-RING FAILED CAUSING ACETYLENE TO ESCAPE AND IGNITE BURNING TOP OF LEFT HAND.
EMPLOYEE REMOVING SCREEN USING AIR WRENCH WHEN A NUT BECAME LODGED IN THE SOCKET OF THE TOOL.WITH THE TOOL STILL IN HAND AND NOT DE-ENERGIZED EMPLOYEE STUCK FINGER IN SOCKET, ACCIDENTLY HIT TR IGGER SCREWING NUT ONTO FINGER. REQUIRED 4 STITCHES FOR TREATMENT OF LACERATION.
