EMPLOYEE WAS OPERATING A TRACTOR IN SHOT AREA WHICH WAS PUSHING ROCK. EMPLOYEE BACKED OVER A ROCK WHICH SHOOK BODY WHEN DOZER CAME OFF ROCK. EMPLOYEE COMPLAINED OF PAIN IN CHEST & RIGHT ARM N OW CLAIMS ALLEGED NECK & BACK STRAIN.

EMPLOYEE WAS STANDING ON A LADDER WHILE REFUELING THE STORAGE TANK OF THE SERVICE TRUCK. THE LADDER WAS SLICK BECAUSE OF RAIN AND SNOW. WHEN HE BENT OVER TO PICK UP THE FUEL HOSE NOZZLE HIS FO OT SLIPPED AND HE FELL FROM THE TRUCK TO THE GROUND. HE COULD NOT HOLD ON THE HANDRAIL TO PREVENT HIS FALL. EMPLOYEE DID NOT MISS WORK UNTIL 3/15/93.

EMPLOYEE WAS OPERAT ING THE ROCK DRILL WHEN EXTRACTING THE DRILL STEEL FROM THE HOLE THE DECK BUSHING CAME UP & OUT OF POSITION THE EMPLOYEE TWISTED THE BUSHING INTO THE PROPER POSITION WHEN H E DID THE BUSHING FELL INTO PLACE & CAUGHT THE EMPLOYEES FINGER BETWEEN THE DRILL DECK & BUSHING 8 STITCHES WERE REQUIRED TO CLOSE THE CUT ON HIS FINGER
