EMPLOYEE WAS CLIMBING THE LADDER TO GET INTO HIS ROCK TRUCK AND HIS FOOT SLIPPED OFF CAUSING HIM TO HURT HIS BACK.
EE WAS RUNNING A DOZER AND STOPPED TO CHECK WATER LEVEL IN THE RADIATOR. WHEN HE TOOK THE RADIATOR CAP OFF, WATER BLEW OUT ONTO BOTH HIS LEGS, BURNING THEM BADLY.
EMPLOYEE WAS OPERATING AN INGERSOLL RAND DRILL. DURING OPERATION, HE THOUGHT HE SAW A WATER LEAK ON THE RADIATOR. HE LEFT THE MACHINE RUNNING, AND PUT HIS HAND ON THE RADIATOR TO DETERMINE IF THE LEAK WAS OIL OR WATER. WHEN HE DID THIS, HIS LEFT HAND CAUGHT ON THE FAN BLADES. THE FAN BLADE AMPUTATED HIS LEFT RING FINGER AT THE FIRST JOINT.
