TWO EMPLOYEES WERE FIXING A PIECE OF BELTING ON THE FINE HEAD UNDERSCREEN CONVEYOR, WHE ASKED IF THEY WERE FINISHED & TO START UP PLANT. ONE SAID YES IT WAS READY. THE PLANT WAS STARTED UNKOWI NG THAT ATHAE INJURED WAS STILL ON THE CONVEYOR BEHIND THE PLANT. OTHER EMPLOYEE RAN OUT & TOLD ME TO TURN OFF THE PLANT. I WENT TO SEE WHAT WAS WRONG. INJURED WAS ALL SCRATCHED UP, ON CONVEY
WORKER WAS IN AN AREA THAT WAS OFF LIMITS WHILE THE LOADER WAS IN THE AREA. WORKER WAS BENT OVER SHOVELING BEHIND THE PRIMARY * GOT HIT ON THE RIGHT SIDE OF HIS BACK & RIBS WITH A ROCK [6"] WH EN THE LOADER WAS DUMPING MATERIAL INTO BIN.
WAS CHANGING OUT ELECTRICAL MOTOR, LIFTING THE MOTOR UP TO THE MOUNT, PULLED LOWER BACK.
SLICED THUMB WITH UTILITY KNIFE.
WAS UNLOADING A STEEL REINFORECED HOSE FROM TRAILER. DISCOMFORT BEGAN THAT AFTERNOON & THEN BECAME EXTREME DISCOMFORT AFTER WORK. THEN PAIN VARIED UNTIL VISITED DR. ON 2/16/93.
