AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #3 ENTRY BELT INTERSECTION AT SPAD 470. THE FALL WAS ESTIMATED TO BE 20'WX20'LX9-12'H. AREA HAD BEEN SUPPORTED WITH 4' FULLY GROUTED RESIN BOLTS. NO INJURIES, VENTILATION WAS NOT INTERRUPTED, FRONTMOBILE BRIDGE WAS CAUGHT. FALL WILL NOT BE CLEANED UP.

EMPLOYEE WAS IN THE PROCESS OF CHANGING OUT LEFT SIDE POT ON CONTINOUS MINER, WAS POSTIONING A CRIB BLOCK UNDER PAN OF MINER WHEN LEFT HAND BECAME CAUGHT BETWEEN PAN ON CRIBB BLOCK. EMPLOYEE S USTAINED FRACTURES AND LACERATIONS TO HIS INDEX AND MIDDLE FINGER.
