AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #3 ENTRY BELT INTERSECTION AT SPAD 470. THE FALL WAS ESTIMATED TO BE 20'WX20'LX9-12'H. AREA HAD BEEN SUPPORTED WITH 4' FULLY GROUTED RESIN BOLTS. NO INJURIES, VENTILATION WAS NOT INTERRUPTED, FRONTMOBILE BRIDGE WAS CAUGHT. FALL WILL NOT BE CLEANED UP.
