On 6/1/12 the employee requested medical treatment, was taken to the company clinic and was prescribed physical therapy for a sprained left shoulder. The employee alleged that he jammed his L-shoulder on an arm rest of a loader while stock piling. He was returned to full duty on 6/1/12.
