Climbing access ladder to grease Extec Screener before startup. Felt something "give" in his the calf of his right leg. Experienced high level of pain and not able to put weight on leg.

Setting up for welding repairs on the top of the extec screener/crusher-pulling welding leads up to the work site. Swung body around strikig forehead against the headblock of the shop truck crane which was holding the repair component in place. Laceration to right forehead requiring sutures.
