The employee reported feeling unwell while operating a front-end loader. EE was transported to urgent care and subsequently referred to the local emergency department, where EE was evaluated and released without a definitive diagnosis. Additional medical testing is ongoing. At this time, the incident is not related to any work activity or workplace condition.

While dumping a haul truck at the crusher, the employee (EE) began feeling sick with chest pains and nausea. EE parked the truck, notified EE's supervisor, and was transported to a medical facility. EE was treated and released the same day with possible heart palpitations and returned to work the next day with no restrictions.
While aligning a ring of anvils on a crusher, the employee (EE) used a chain that caught on the feeder, creating a pinch point. In trying to free it manually, the EE's finger was pinched, causing a crushing/laceration injury that required sutures. Contributing factors included the jammed chain, hand clearance, and exposed pinch point.

Mobile Equipment Technician was repairing a drill. While removing the repair parts from the shipping box EE sliced EE's right index finger with a box cutter.
EE tripped over the skid steer bucket as EE was stepping back after switching the attachments from the bucket to the broom. EE sustained a fracture to EE's right pinky finger in the process of breaking EE's fall. The incident did not involve any equipment or compliance issues. It also did not involve job skill, proficiency, training, PPE or protective devices.

employee was carrying toughing roller and stepped on a rock, lost balance and dropped the roller on their rt foot. EE continued to work stating they did not need to seek medical treatment. EE sought medical treatment the next day and was taken off work for rt foot pain and can return to work on 09/09/2019. EE has returned to full duty
While exiting the water truck, EE felt a 'pop' in EE's right leg and pain in EE's ankle. EE fell to the ground between water truck and maintenance truck. EE got up and leaned against water truck but then collapsed to the ground, hitting EE's head on the passenger side running board of the maintenance truck. EE was taken to the hospital and found to have a sprained ankle.
Employee was cleaning belts and catwalks from the recent snow event. EE slipped on catwalk while cleaning, caught self on the hand rail. EE felt pain under shoulder blade. EE suffered chest wall strain.
Removed top rollers on a PC650 utilized a bottle jack to get some slack for the placement of the roller. That is when the jack slipped catching the employees hand

Employee was picking up the center chute for a screening unit. When the employee picked up the unit, the employee suffered a left shoulder strain.
Employee stated that EE was backing up to stock pile to dump load. As EE raised the bed, EE felt the truck going onto its drivers side. After it was on its side, EE called for help on CB and was assisted by two employees to exit cab.
Employee cut the palm of right hand while handling a piece of metal.

Was dismounting a Euclid truck utilizing the steps and hand rails when his right foot touched the ground his ankle became twisted causing a twisted ankle sprain.
Employee walked into office complaining of tightness in chest: She stated that she had been driving from our Rocky Mount location to our Roanoke location, when about half way between she begin to feel sick and considered pulling off the road. She drove on to the Roanoke Va office.

Employee was helping install a culvert pipe near the Scales Area when he bent over to pick up a chunk of mud which was about to fall in the ditch. While reaching for the mud chunk, he felt something pull in his back.
Employee was training new employee when drill steel became dislodged from carousel and struck the employee. The employee recieved two mild compression fractures in his back.
Employee was nauseated and later complained of pain in his left arm.
Employee was working in the 8,s Bin reparing holes in a chute under the screen. He ask another employee to cut him some small pieces of metal to use to patch the holes. The employee cut the metal droped one piece into the bin when the employee doing the welding picked up the second piece was droped strikeing the second finger of the right hand.

Customer's truck had previously delivered a load of stone to a project. The driver came back to quarry to get another load. While waiting to get loaded he attempted to remove a rock from his tailgate. The automatic closure was still engaged and he caught finger in the tailgate latch.
Employee was training another employee which required both of them to be in the cab of the vehicle. The employee stated he was required to sit in an area that had no seat. This plus the movement of the truck caused back sprain. This employee had a similar injury prior on 10/16/06.
Employee was lowering object into 12ft Classifier using chain hoist when ratchet handle flew back and struck his hand against piece of metal
Employee was attemting to remove ramps from trailer when hinge rod slipped striking his right hand.
Employee was lifting screen housing out of wash take and placing it on work bench when housing slipped out of his hand causing his finger to be caught between tank and screen.
Employee was dismounting from an R-60 Euclid Truck on grase pad. Left foot slipped thru bottom swing step, causing employee to fall, bruising calf area of left leg.
After climbing stairs to controls on loadout bin I slipped on the ice and twisted my leg.

Employee not sure if air blast from oxygen side of torch caused a piece of dirt or slag to get into eye. Physician removed object with no problem. This accident was not reported to supervisor until 02/03/2007.
Employee was cleaning off catwalk when he decided to kick a large, what he thought to be a mud ball. When he kicked at the mud ball he discovered that it was a rock. He was placed off balance when the rock did not move thus straining his back.

IN THE GARAGE, INT HE PIT BENEATH WATER TRUCK REMOVING 2 PC DRIVE SHAFT. THE 2 PC DRIVE SHAFT SEPARATED THE UNSECURED PC FELL AND STRUCK EE ON R. ANKLE. EE DID NOT THINK TOO MUCH ABOUT IT AND DID NOT REPORT OR ASK FOR MEDICAL ATTENTION. IT WAS FRI. ANKLE SWELLED UP OVER WEEKEND WITH PAIN. MISSED WORK ON MONDAY. REPORTED.
