WHILE GRINDING METAL, SPARKS WERE NOT DIRECTED AWAY FROM EMPLOYEE'S BODY. DOCTOR'S REPORT STATES EYE WAS SWABBED WITH Q-TIP, EMPLOYEE STATED HE USED TWEEZERS TO REMOVE PARTICLE. WEAR SAFETY SHIELD OVER SAFETY GLASSES WHIL GRINDING. THIS REPORT BEING SENT DUE TO PART 50 AUDIT.

WHILE CLEANING RECOVERY ROOM FLOOR WITH WATER HOSE, THERE WAS A BACK FLASH OF CAUSTIC SODA SOLUTION DOUSING RIGHT LEG OF EMPLOYEE. EMPLOYEE HOSED DOWN HISLEG AND BOOT BUT WAS UNAWARE THAT SOLU TION RAN INTO HIS BOOT SITTING ON ANKLE.
