PULLING TRANSFORMER LEVER TO START GYRO 440 VOLT -- CONTACT NOT PROPERLY CONNECTED. THREW BODY 8-10 FT. ACROSS ROOM. LANDED ON TABLE AND CONVEYOR ON FLOOR.
WAS LIFTING LID OFF THE HYDRAULIC TANK SET IT ON THE TIRE, WENT TO PUT THE LID BACK ON, IT SLID OFF AND HIT HIS LEFT FOOT.
OPERATING FRONTEND LOADER.HIT POTHOLE AND WAS BOUNCED AROUND IN CAB.HAS SORE BACK.WASNT REPORTED TO EMPLR UNTIL 8-4-88 BECAUSED HE THOUGHT IT WOULD GO AWAY BY ITSELF.
EMPL WAS CUTTING BELTING WITH KNIFE & KNIFE SLIPPED CUTTING PALM OF HIS LEFT HAND.
