Employee was exiting the 992 C loader and missed the last step causing him to twist his left ankle. Initially treated in the ER, was released without restrictions. He requested medical treatment again, 11/4/10 was given an steroid injection to the ankle, medical records recieved 11/18/10 notifying us of the injection.

EE WAS ENTERING THE CAB OF A 773 CAT HUAL UNIT WHEN HE SLIPPED AND FELL APPROX. 7 FEET AND STRUCK HIS SHOULDER THE EE WAS NOT HOLDING ON TO THE HANDRAILS PROVIDED AND HAD NOT CLEANED OILER FRO M HIS BOOTS BEFORE MOVNTING THE HAUL UNIT.
EMPLOYEE WAS USING A PRY BAR WHEN THE BAR BROKE. THIS CAUSED THE EMPLOYEE TO FALL HITTING HIS ARM AND SHOULDER.

EMPLOYEE WAS ASSISTING THE DRILLER WITH MAINTENANCE ON DRILL AND WHILE LOWERING THE BOOM, THE DUST CONTROL SUCTION HOSE CAME OFF STRIKING THE EMPLOYEE ON THE TOP OF HIS HARD HAND. THE EMPLOYEE SUSTAINED A CONTUSION AND STRAIN OF THE CERVICLE SPINE.
EE WAS ASSISTING A TRUCK DRIVER IN UNLOADING A SHIPMENT OF SCREEN CLOTH THE FREIGHT DRIVER JERKED THE SKID AND THE SCREENS SHIFTED PINCHING THE EE S RIGHT HAND RESULT WAS A LACERATED MIDDLE FI NGER.
