An accident occurred this day at 8:36 a.m. when the operator of the wheeled front end loader (Komatsu WA500 #YL1) was ejected from the cab and landed on the ground next to the rear tire of the loader. The operator was in the process of loading an off-road haul truck at the shot material stockpile, when an approximately 1.75-inch hydraulic line for the bucket lift cylinder failed.

The employee was trimming oak trees out of planted pine trees with a set of lopers and a chain saw. He let the chain saw get into the lower left front of his leg, causing a 1&1/2 inch laceration.

EE and co-worker were attaching a Batwing Mower to a 5400 John Deere Farm tractor. EE had put on the PTO Shaft and pulled backward on the shaft to make sure it was locked on. The shaft slipped off and dropped about 6", mashing and cutting the tip of his little finger on his left hand. He was transported to Jackson Hospital by a company vehicle.
Four men were on the ground prying with a 16 ft bar trying to turn a 36 in crusher drive pulley. The prybar was placed behing a bolt and against the pulley. The bar slipped off the bolt and the bar moved toward injured and he started to fall backward. He rotated to catch on his hands and his right leg came across a bar laying on the ground.

WORKING ON AN AIR OPERATED GREASE PUMP. GREASE PUMP HAD AIR LEAK. PLACED HAND NEAR TO SEE WHERE AIR WAS COMING FROM AND THE AIR PISTON CAME DOWN AND MASHED FINGERS. THE AMPUTATION WAS JUST BEL OW THE NAIL OF MIDDLE FINGER, 1ST KNUCKLE.

EE WAS REPLACING A V-BELT ON A LINE (CONVEYOR) THAT FEEDS THE HAMMER MILL AND HE PUSHED TOO HARD. FELT PAIN IN BACK. PER EE. NOTE: THIS EE HURT HIS BACK IN JUNE OF THIS YEAR AND IS STILL ON LI GHT DUTY FROM THAT ACCIDENT.

THE EMPLOYEE SETS IN A BOOTH TO WATCH THE PLANT. TO GET TO THIS BOOTH, THERE ARE STEPS TO WALK UP AND DOWN. HIS FOOT SLIPPED ON THESE STEPS, CAUSING HIM TO FALL.
