I WAS TRAMING ROOF BOLT HEAD & CATCHING MY HAND IN BETWEEN IT AND STEERING LEVER
A KETTLE BOTTOM THAT HAD GONE UNDECTECTED FELL ON THE OPP OF THE MINER. BENDING HIM OVER SO FAR AS TO BREAK A BONE IN THE LOWER OF HIS BACK.
THREW BELT CLAM1 ACROSS BELT WHICH WEIGHED 30 LBS AND STRETCHED MUSCLE IN LOWER BACK.
WHILE SETTING THE JACK THE BIT PULLED INTO THE TOP ON THE OUTSIDE OF KETTLE BOTTOM CAUSING IT TO PULL LOSE & STRICK HIM ON THE ANKLE
WHILE PUTTING OIL IM MINER VICTIM WAS STRUCK FROM BEHIND WITH A KETTLE BOTTOM STRIKING HIM ON THE RIGHT SHOULDER CAUSING BICTIM TO BEND OVER TO FAR AND BREAK THE TOP RIB ON THE RIGHT SIDE.

WENT TO POWER CENTER TO KNOCK BREAKER ON HIS WAY BACK THE HIT SOLID & STARTED BACKING.
HE HAD GIVEN ORDERS TO TIGHTEN UP THE BELT WITH A SHEAVE BLOCK AND CHAIN WHEN THEY PROCEEDED TO DO THIS THE HOOK ON THE SHEAVE BLOCK BROKE AND PART OF THE SHEAVE BLOCK HIT HIM IN THE CHEST
WHILE CRAWLING UP THE BELT HEADING A ROCK 20" X 15" X 1 1/2" FELL ABOUT 6" AND HHIT HIM IN THE BACK
