Employee was turning the water valve at the plant when his hand slipped off the end of the handle and a small piece of metal wire went into his right middle finger.

Employee was repairing the lining in the trough under the wash screen when he picked up a bucket of cement and strained/sprained his back.

Working under haul truck and upon standing employee felt pressure in his left knee.

EE was assisting to assemble a HP 500 crusher, tightening the screw down, using a shop crane hoist and drive. The drive went under the screw and employee's right middle finger was contacted between the wrench and the drive, causing a laceration to the finger.

The employee was changing skirt rubber on the surge tunnel conveyor.He was carrying a piece of angle iron into the tunnel, lost his footing and fell backwards. It was propelled forward as he fell backwards. As the angle iron fell, the end closest to the employee struck him on the right foot , penetrated his boot and lacerated the inside of his foot.

EE's left hand finger was crushed between pry bar and a metal skirt, while using a pry bar to clear a rock out of the plant feeder at conveyor #2, receiving ten stitches.

EE sprained his right knee after his foot slipped off the 600 Komatsu Loader step ladder, falling 6-7 feet and landing on his right foot.

The EE's left middle finger was fracture and lacerated, with three stitches, after removing bolts from an planetary assembly spindle that fell on it. EE was not wearing required gloves.

EE sprained his back while reaching to catch himself after losing his balance when debris entered his face and underneath his safety glasses. This happened while using a pressure washer to clean a 988F Cat Loader.

Contused two left fingers when a piece of drill steel fell on his hand.

EE WAS CLEANING AND INSPECTING THE SHOP GREASE PUMP WHICH HAD BEEN LEAKING GREASE AROUND THE PISTON. HE TOOK THE GUARDS OFF TO FIND THE LEAK, WHEN THE PISTON GRABBED THE RAG IN HIS HAND. WHEN ATTEMPTING TO FREE THE RAG THE PISTON CAUGHT HIS FINGER, LACERATING A 2" LONG PIECE OF SKIN.

RAIL CAR WAS LOADED AND THE DOOR POPPED OPEN; REMOVED 20 TONS OF ROCK; HOOKED A BAR IN THE DOOR ; HOOKED A CHAIN HOIST TO A BAR AND THE RAIL CAR TO PUSH THE DOOR SHUT; HOOK ON THE END OF CHAIN HOIST SLIPPED OFF THE BAR, CAUSING THE CHAIN TO HIT VICTIM IN THE LEG.

HE WAS REMOVING AN OIL LINE THAT BROKE LOOSE PINCHING HIS RIGHT RING FINGER. ON IMPACT, HE JERKED HIS ARM BACK STRIKING A TRANSMISSION BRACKET WITH RIGHT ELBOW. IMPACT BROKE A PIECE OF HIS ELB OW LOOSE (A FRACTURE EXISTED FROM A PREVIOUS INJURY).

EMPLOYEE HAD A PERIODIC SCREENING CHEST XRAY READ BY 3 B READERS USING THE ILO PROCEDURES THE XRAY WAS CHARACTERIZED AS BEING CONSISTENT WITH PENEUMOCONIOSIS BY TWO READERS WITH SCORES OF 1/1 & 0/1 DAMAGE OF IMPAIRMENT CANNOT BE DETERMINED BY READING CHEST FILMS ONLY THIS RESULT IS BEING SUBMITTED TO SATISFY MSHA PROGRAM POLICY LTR P92-II-2

POSSIBLE PNEUMOCONIOSIS.

DRILLER WAS ATTEMPTING TO RE ASSEMBLE A NEW HAMMER AFTER LUBRICATING THREADS ON BAK HEAD WHILE HELPER WAS HELPING HAMMER UPRIGHT APPENTLY BULL LEAK MISLINGEREWD WITH HAMMER THE EJECTING HAMMER AND STRICKING HELPER ON LEFT SIDE OF HEAD. RESULTING IN FRACTURED SKULL AND OTHER CONTUSIONS TO LEFT EAR AND HEAD.

INJURED WAS STEPPING OFF THE DRILL IN THE PIT WHEN HE STEPPED DOWN HIS RT FOOT HIT A ROCK AND CAUSED HIM TO FALL

AT THE END OF T2E WORK DAY-SHE WAS CLIMBING DOWN FROM HER TRUCK AND SHE WAS HOLDING ON TO THE BAR THAT GOES ACROSS THE HOOD.SHE STEPPED DOWN ONTO THE GROUND AND FELT A SHARP PAIN GO UP FROM HE R LEFT ARM TO H9R NECK.

SHAKER SCREEN 40 HP MOTOR KNOCKED BREAKER OUT WHILE IN OPERATION.INJ WENT TO MOTOR TO DETERMINE PROBLEM-NOTHING WRONG SO HE WENT TO BREAKER SWITCH.USING TEST METER CHECKED 480V GOING TO 100 AM P BREAKER SWITC2 WHEN BREAKER EXPLODED BLOWING MIDDLE LEAD OFF.INJ WAS WEARING SAFETY GLASSES AND HARD HAT.BOTH HANDS WERE INSIDE BOX.RAN TO WATER FOUNTAIN.EVIDENT THAT FACE,HANDS BURNED.

HOOKING UP AIR 7OMPRESSOR TO TRUCK AND PULLED MUSCLE IN HIS BACK.
