See Attached Letter Per Attachment Condensed: EE was splitting a piece of stone with a hammer and chisel and a small piece flew off and struck EE in the genital area. Injury was not reported to company until 6/21. EE on pain meds and is not working.

EE WAS WALKING FROM ONE QUARRY SITE TO ANOTHER WITH TWO OTHER EMPLOYEES. HE WAS WALKING DOWN A DIRT ROAD WHEN HE STEPPED ON A SMALL ROCK THAT SPUN OUT FROM UNDER HIS FOOT CAUSING HIM TO SLIP S FN SPRAIN HIS LEFT ANKLE. APPARENTLY ACCIDENT STRETCHED AND SPRAINED A TENDON (SEE DR.'S NOTE)
