Employee was assisting two others with a wall liner change in the crusher. EE was using a pry bar to line up the holes of the liner when the bar slipped and hit EE in the mouth causing a busted lip and chipped tooth.

Employee was assisting others with a drive shaft installation on a washing screen. While he was tightening a bolt, the chain holding the counterweight slipped off, allowing the counterweight to rotate clockwise pinching his finger between it and the impact wrench. This caused him to receive a laceration and fracture to his left index finger.

On August 7th, employee slipped on step while entering bobcat, causing a minor laceration to his leg, not requiring medical attention. On August 16th employee stated the wound was not healing and requested medical attention. He was treated for an infection and returned to full duty. This incident is being reported within 10 days of employee seeking medical attention.
